The ucontext_t and mcontext_t structures are added here from the obsolescent <ucontext.h> header.

I have updated the html to remove the automatically generated link.
I will need to rebuild the download bundles.

 This has been fixed in the online pubs but the PDF needs to be recoded in these places to avoid this problem in the future.
